Driver Documents:

* Upload a clear picture of yourself
* Remove any hats, caps, sunglasses or hoodies (exceptions: eyeglasses or religious headwear)
National ID* Name matches other submitted documents
* Photo matches profile picture
* Driver must be at least 20 years old but not older than 70 years old
Professional License (Front)* The license type must be a Professional Driver’s License
* The name and date of birth must match with National ID.
* The license must not expired
* A convex mark must be visible on the document
* The person must match the photo on the license
Professional License (Back)* The inspection date must not be expired
* For drivers under the age of 60, a vehicle inspection must be completed every 3 years from the date of issue, before the listed inspection date
* For drivers aged 60 and above, a vehicle inspection must be completed annually from the date of issue, before the listed inspection date
Taxi Driver Certificate* Please ensure both the main and duplicate license are visible in the same photo.
* The “Expiry Date” on the secondary document must be valid
* The “inspection date” on the primary document must be valid
* The ID number on the primary document must match with the ID card.
Fleet Contract* The information on the document must match with National ID
* The issue date must be in the past or the day the document is uploaded
* Drivers will need to upload the contract with (1) National ID (2) Signature Consent Form (Provided at Fleet), so please remember to bring your National ID to your fleet.

Car Documents:

Vehicle Registration* The vehicle must be registered as a commercial passenger vehicle
* The designated inspection date must not be expired
* The vehicle must be within 12 years of age
* Vehicles from 2012 may be used until 2025/12/31
* Vehicles from 2013 may be used until 2026/12/31
* The vehicle registration must match with compulsory insurance
* If the service company or lessee is indicated, the registration must include the note: “Self-provided vehicle and driver: OOO”
Compulsory Insurance* The insured party must match the vehicle owner on vehicle registration
* The information on the document must match with the vehicle registration (e.g., vehicle type, license plate number, etc.)
* The insurance coverage period must be valid
Passenger Insurance* The insured party must match the “vehicle owner” listed on the vehicle registration
* The license plate number must match the “license plate number” on the vehicle registration
* The “insurance coverage period” must not be expired
* The insured amount must be at least NT$5M per person

(Coverage)
* Third-Party Liability Insurance – Minimum NT$2 million for bodily injury per person
* Third-Party Liability Insurance – Minimum NT$4 million total per accident
* Third-Party Liability Insurance – Minimum NT$200k for property damage per accident
* Driver Injury Insurance / Employer’s Liability Insurance – Minimum NT$2 million per injury and per accident
* The coverage of Driver Injury Insurance / Employer’s Liability Insurance must include compensation for “traffic accidents.” If it only covers “single-vehicle accidents,” it does not meet the requirement
* If the coverage of the third-party liability insurance is insufficient, an excess liability insurance policy must be provided to cover the shortfall (i.e., any uncovered bodily injury or property damage amounts mentioned above)
* For example: If drivers third-party liability insurance coverage is NT$1 million per person for bodily injury, NT$2 million per accident, and NT$100,000 for property damage, then drivers excess liability insurance must cover at least NT$2.1 million = (4M- 2M) + (0.2M - 0.1M)

* All fields on the document must be fully completed and consistent with the driver’s other documents (such as name, license plate number, engine number, ID number, etc.)
* The certifying organization must be a MPT fleet authorized to operate on this Bolt
* The document must be a photo of the original copy
